Entry Forms & Competitor Declarations
Purpose
After completing registration the last stage is to generate an actual
Entry Form that can then be printed. Producing a physical record of
the entry that can then be used for a variety of purposes.
What It Contains
The entry form details all of the person's basic information like name, address, date of birth,
as well as their travel and accommodation information. Also detailed for information or an
emergency are their emergency contact, telephone numbers (at home and during the games) and also
any medical / dietary / special requirements they may have.
Competitors
For competitors the Entry Form also details all of the events they are entered into along with
their record / ranking for that event. A further page is also produced containing the
Competitors Declaration that must be signed by the competitor.

In order to help the collection of all the information there is a PDF and Microsoft Word blank
Entry Form that can be downloaded from the site and handed out to relevant people to fill in.
